Before Creating Your Account

You will need the following information to allow for accurate completion of your online registration:

• Student’s Basic Medical History Information
• Health Insurance Company Information and Policy Number
• Doctor and Dentist Contact Information
• Hospital Preference

Creating an Account

FinalForms sends an email to parents and students with steps to create a FinalForms account. The student must be enrolled at West Seattle High School before an account creation email is sent. View the FinalForms Parent Playbook for assistance creating both student and parent accounts.

Registering a Student

1. Visit FinalForms and login under the parent icon.
2. Select the sports for which you would like to register the student. Your selection can be changed at any time prior to the registration deadline.
3. Click incomplete forms. Complete each form and sign your full name in the Parent Signature field at the bottom of the page. After signing a form, click submit form and move on to the next form. You will see  a “Forms Finished” message when all forms are complete.
4. An email will automatically be sent to your student prompting them to complete the forms requiring student signatures.
5. If you have additional students, click My Students and repeat steps 3 – 6 for each student.
6. If you need to update your account information including contact information, medical history, or address, please do so as soon as you can.

Medical Insurance for Athletic Participation

Medical insurance is required to participate in high school athletics.

If you do not have medical insurance, we provide information regarding student accident and health insurance coverage administered by Myers-Stevens & Toohey, Inc. Coverage is available for:

• Interscholastic Tackle Football
• School-Time Accident Plan
• Full-Time 24/7 Accident Plan
• Dental Accident Plan

These low cost plans are used by many school districts across the state. Although they are not the only provider of this type of coverage, they have been recommended by our Washington Schools Risk Management Pool.

Physical Participation Forms

Please have your medical provider fill out Seattle Public School’s physical clearance form. Any other forms will likely not be approved!

Upload your student’s current physical form into their Final Forms account. Don’t know if the physical is current? Check The Source if you were enrolled in a sport at West Seattle HS during the 2024-2025 school year or contact the Athletic Secretary Sarah McBride at smmcbride@seattleschools.org.

Physicals are valid for 24 consecutive months.

• The Medical Eligibility document (Page 3) should be the only item submitted to your school.

• The History document (Page 1) and the Exam document (Page 2) should be completed and remain with your provider.

NOTE: Physical Forms must be signed by physician, stamped with clinic name, and include the date of exam.

Schedule Your Free Physical at WSHS!

During the school year, students may access free health center support at our Neighborcare Health Center on the WSHS campus. An appointment for completing your student’s physical clearance can be made in-person or online. Access Neighborcare Health Center contact information here.

Athletic Registration Fees

Purchase your ASB sticker online here for the 2025-2026 school year when participating in a sport at WSHS. If you wish to pay in person, find information about how to pay in-person here. Upload a copy of the SchoolPay receipt to your FinalForms account.

All students participating in WSHS Athletics are required to purchase an ASB sticker. These funds directly support the cost of athletic programs: game transportation, equipment, uniforms, and more. Athletes must provide proof (receipt) for final athletic clearance.

Students Not Enrolled Full-Time at WSHS

Students not enrolled full-time at WSHS will need to fill out additional forms and provide grades from previous school.

Students are required to complete the following:

• Enrollment with SPS as an athletics-only student. Use this athletics-only checklist to walk you through the enrollment process.
• WIAA contracts. You can find more information on the WIAA website.

Running Start

If you are enrolled in Running Start, please complete the WIAA Running Start Contract on the WIAA website. To find it, scroll down to the Maintaining Academic Eligibility section and select the WIAA Running Start Contract. Return the Running Start contract and your class schedule to the Athletic Secretary by uploading it to your Final Forms account.

Important! Please confirm your neighborhood school by using the “Address Look Up Tool”. Provide proof of WSHS ASB fee purchase and your most current semester grades to the Athletics Secretary by uploading it to Final Forms.

Off-Campus Classes and Private School

If you are enrolled in any off-campus classes, please complete the Alternative Education School Contract on the WIAA website. To find it, scroll down to the Maintaining Academic Eligibility section and select the Alternative Education School Contract.

Important! Please confirm your neighborhood school by using the “Address Look Up Tool”. Guardian/student completes SPS Online Enrollment Once enrolled, please submit it to the Athletics Secretary by uploading it to Final Forms.

Home School

If you are home schooled, please complete the Home Based Education School Contract on the WIAA website. To find it, scroll down to the Maintaining Academic Eligibility section and select the Home Based Education School Contract. Print your class schedule and return to the Athletics Department.

Grades

Grade checks are run throughout each sports season. Please provide a copy of previous semester and most recent quarter grades. Failure to provide grades may make you ineligible for competition. If you attend any other school for ANY classes, you must provide the Athletic Office with your grades from that school.

Transfer and Exchange Students

Students who are transferring to WSHS or are exchange students will need to fill out additional forms and provide grades from previous school.

Students are required to complete the following:

• A WIAA contract. You can find more information on the WIAA website.

If you are transferring schools to WSHS, please complete the Before You Enroll Transfer Checklist to evaluate your student’s potential eligibility status. To find it, scroll down to the Changing/Transferring Schools section and select the Before You Enroll checklist. Once completed contact Athletic Director Corey Sorenson at cjsorenson@seattleschools.org and include the completed checklist.

Grades
