    Pfizer vaccination event for West Seattle High School students.

    SPS is partnering with the City of Seattle, Public Health – Seattle & King County, and Seattle Fire Department to host a COVID-19 vaccination clinic for students 12 and older at West Seattle High School.

     

    Location at the school: West Seattle High School Courtyard

    • Tuesday, May 18, 9 a.m. - 12:30 p.m., 1st dose
    • Tuesday, June 8, 9 a.m. - 12:30 p.m., 2nd dose
    • Friday, May 21, 2 - 5:30 p.m., 1st dose
    • Friday, June 11, 2 - 5:30 p.m., 2nd dose

    If the student is under 18, they will need to bring the signed parental consent form. Consent form links are listed below or available in the school main office.

     

    Families can return consent forms via:

    • Print, sign, and return paper form with student attending clinic
    • Accompany student to clinic to give verbal consent (If clinic is outdoors)
    • Email the main office if you need to give electronic consent.

     

    We are excited to offer a vaccination clinic at our school. If your student is not able to participate in this one, there are many other opportunities to be vaccinated.

     

    Consent Forms:
